Health History Forms

The web-based medical forms that come provided with the medical module are called "Health History Forms." Parents complete these forms by typing their responses into each form, straight from the parent dashboard.

By default, the health history forms that are included with the Medical module appear as 4 separate web forms for the parent to complete. However, each camp can customize their health history forms via their Admin settings. To learn more about customizing health history forms, click here.

To learn more about the parent's process for completing health history forms (the questions included on these forms), please read our corresponding support articles by clicking here.

Printed Medical Forms

If needed, camps can also create "printed" medical forms to allow parents to provide copies of medical documents to your camp (by uploading a file from their device).

Printed medical forms will appear beneath the Health History Forms (with a "Printed form" label beneath the form title).

Printed Medical Forms

Parents can access your printed medical forms within the "Medical Forms" section of the forms page, beneath your Health History Forms.

Medical Forms Section

In the example above, Camp Firewood created a printed form (as indicated by the label beneath the form name) to allow parents to upload an immunizations file (e.g. a scanned copy of the camper's immunizations sheet from the doctor).

Please note that no medical forms will display until the camper has begun the enrollment process for the corresponding year.

Printed forms are completed by clicking the name of the form, uploading the necessary file, and submitting the form.

If you upload a blank form that parents need to download and complete, a 'download' button will also appear on the screen pictured above, allowing parents to download the blank form first.

To create printed medical forms, go to the "Medical Management" section of the Medical module dashbord.

How to Create a Daily Health Screening System

The built in Health Forms do not have a built in daily screening system. If you need to collect and enter the data from ongoing health screenings, we recommend using the following features:

  1. Create a camper web form (per session).
    • Hover over the green Campers button on the top navigation bar > click on Camper Forms.
    • Create a separate form per screening; or, setup multiple screenings onto the same form (e.g. for daily screenings, create a separate form per day or create a separate form per week with multiple days on each week's form).
    • If needed, forms can be made admin-only (i.e. not accessible by parents) if they're set so "Active?" = "No."
    • To learn how to create a web form, read our support article.
  2. Add Notes on each camper's profile.
    • To create a new category for containing your screening notes, go to Admin > Campers tab > Notes > Add category.
    • To enter a note for a camper, go to the camper's profile page > Notes tab > select a category > Add note.
  3. Use session-specific Custom Fields.
    • To create custom fields, go to Admin > Campers > Custom fields > Add new custom field. (Scope should be "Session").
    • To enter custom field data for a camper, go to the camper's profile page > 2020 tab > select the sub-tab for the relevant session > select the Custom Fields sub-tab > double click 'edit...' > enter data > click OK to save.
