By default, "Health History Form, Part 3" collects medications and forbidden over-the-counter medications.
To customize this form, navigate to the "Health History Form Configuration" section of the Medical settings within the Admin tab. To learn more, click here.
Form Header
Custom instructions added to "Health History Form, Part 3" will appear just below the name of the form, as pictured above.
Medications
- Custom instructions added to the Medications section will appear here.
- This text is built into the medical form and can't be updated or removed.
- Click to provide additional medications.
- Medication details are entered here.
- You can configure which medication delivery times appear in this list via your camp's "Medical Deliveries". Click here to learn more.
Forbidden Over-the-Counter Medications
- Custom instructions added to the Forbidden OTC section will appear here.
- This text is built into the the medical form and can't be updated or removed.
- Forbidden OTCs are selected by checkmarking the corresponding checkbox.
Note that the list of over-the-counter medications comes provided with this form, but you may edit this list via your camp's "Medical Dropdown Lists". Click here to learn more.
Sign & Submit
Each page of the health history form must be signed before submitting.
- The person completing the form must type their full name as a signature.
- The form is saved/submitted to your camp by clicking the 'Submit Form' button at the bottom of the page.
