How do I create a form for uploading a copy of a physical document?

Create a "printed"-type form. 

CampSite offers two types of forms - "web" or "printed." Web forms are completed by typing responses directly into the Dashboard, while printed forms are completed by uploading a file from the user's device (e.g. a PDF document, JPG image, or Word doc).

When creating a printed form, you have two options: provide a blank form for the user to download, complete, and upload back to you; or, refrain from providing a blank form and just prompt users to upload their own file to you (e.g. a scanned copy of a report card; immunization records; their proof of income; etc.)
