This article will show suggestions and examples of how CampSite can be used to gather form information about campers. Camper forms gather information about each individual camper who is part of a family unit.

Permanent Forms
Becuase Permanent Forms are full of information that is gathered only one time (the first time the family creates their account and enrolls a camper), Permanent Forms should be for information that is unlikely to change in the future.
One example of a Permanent Form is a Photo Release Form.
A Photo Release Form would ask if a camp is allowed to take photos or videos of a camper. It may also ask if those photos and videos can be posted to a camps website, or even to the camps Camp Today Blog within CampSite.
Having a dropdown option for parents to select "Yes" or "No" tends to be easiest here.
It is also common that a "Signature" questions is added using the "Small Text" field. This gives parents/guardians the option to type in their name and electronically sign the form.
A date question is also common in a form like this. This will ask parents/guardians to select the date that they are filling out the form before submitting.
Yearly Forms
Yearly Forms will be full of information that needs to be gathered for each camper each year that they enroll in a camp. This will be information that is likely to be consistent for the entire calendar year.
One sample of this type of form is a Camper Information Form.
A Camper Information Form could be full of information that is needed for this years placements or activities. Asking for a campers interests, skills, habits, or needs could assist camp staff with placing that camper in a bunk, group, or activity that they can thrive in for the camp season.
This form best utilizes "Large Text" and "Small Text" answer type questions.
Session Forms
Session forms are unique to each session from Step 2 of your Setup Wizard. These types of forms will only display to those campers who are enrolling in the sessions that the form is created for. This is a way to restrict who needs to fill out a form and who can bypass it completely.
One example of a Session Form is a Field Trip or Activity Waiver.
A Field Trip or Activity Waiver is best used as a Session Form when only certain sessions out of your camp season are going on a field trip, while other sessions are not. For example, perhaps your session called "Art Camp" has a field trip to the local museum each year, while your session called "Math Camp" does not go off campus during their camp season.
Having a dropdown option for parents to select "Yes" or "No" tends to be easiest here so parents can approve or not approve their campers participation in trips or activities very clearly.
It is also common that a "Signature" questions is added using the "Small Text" field. This gives parents/guardians the option to type in their name and electronically sign the form.

Daily Forms
Daily forms are form information that is filled out by parents/guardians each active day of the session that the camper is enrolled in. Just like Sesssion forms, these forms will only display to campers who are actively enrolled in the sessions that they are made for. Unlike the other form scopes, Daily Form information is only reportable and active for the calendar date it is filled out for.
For this reason, a common Daily Form type is a Lunch Choice Form.
The Dropdown List option is best in this type of form to display the options a camper will have for the day.
It is also common to ask for food allergies or needs in a form like this using a "Large Text" field to describe allergies or needs if necessary.
