Permanent Forms
Becuase Permanent Forms are full of information that is gathered only one time (the first time the family creates a family profile and enrolls a camper), Permanent Forms should be for information that is unlikely to change in the future.
One example of a Permanent Family Form is a Household Information Form.
In a Household Information Form could gather information such as additional languages spoken at home or permanent emergency contacts (other than Parent 1 and Parent 2) who may live with the camper(s) and should be contacted in case of emergency.
Yearly Forms
Yearly Forms will be full of information that needs to be gathered for the family unit each year that they enroll their campers in a camp session. This will be information that is likely to be consistent for the entire calendar year.
One sample of this type of form is a Financial Aid Application.
A Financial Aid Application type form would ask questions about the family's income and enrollment status in assistance programs. This would typically be an optional form for families to fill out each year.
